My Roswell provides important information from your medical record for you to review with your health care providers. You may also use My Roswell to ask questions related to your health, your appointments, and your account. You may give access to Authorized Users, and this access can be discontinued, if you change your mind. If you allow access to your information by your Authorized Users, you do so at your own risk. Do not use My Roswell for urgent health needs. For emergency care, either go directly to the emergency department of a local hospital or call 9-1-1. For non-emergency care, call your health care provider directly. If you are calling after clinic hours, call 1-716-845-2300.

Terms and Conditions
Information provided through My Roswell is provided under the terms and conditions provided below. If you are a patient of Roswell Park Cancer Institute (RPCI) and are accessing your own records, you may give access to your Authorized Users. In the Terms and Conditions below, the term “Users” includes patients, their Authorized Users, and consulting health care providers accessing My Roswell. BY USING MY ROSWELL, EACH USER AGREES TO COMPLY WITH THE FOLLOWING TERMS AND CONDITIONS. THESE TERMS AND CONDITIONS MAY BE CHANGED FROM TIME TO TIME, AND USE OF THE SITE IS CONDITIONED ON USER’S ACCEPTANCE OF THOSE TERMS AND CONDITIONS AS CHANGED. USER AGREES THAT RPCI IS NOT RESPONSIBLE OR LIABLE FOR ANY DIRECT OR INDIRECT CLAIM, LOSS OR DAMAGE RESULTING FROM HIS OR HER USE OF THE MY ROSWELL SITE.
The information provided on My Roswell includes general health information and some of the information in the patient’s RPCI medical records, as well as a means to communicate non-urgent questions to RPCI personnel. The information on My Roswell is made available for patients and their Authorized Users to share with health care providers now and in the future to improve understanding of health conditions. The information is not intended as a tool for self-diagnosis, is not a recommendation of a specific treatment plan or healthcare provider, and is not a substitute for proper medical advice, diagnosis or treatment. Your doctor or other licensed health care provider is in the best position to assess individual health care needs and use of this site cannot replace medical consultation with a qualified health or medical professional. Information on My Roswell is provided “as is” without warranty of any kind.
The information displayed in My Roswell is not the complete medical record. Patients who need complete copies of the medical records must contact RPCI’s Health Information Management Department and complete a release of information request form.
Site Use and Security
Users are granted a limited, revocable license to access and make use of the My Roswell site only for personal use in accordance with these Terms. Users may not: (i) create derivative works using the My Roswell site or its contents; (ii) use data mining tools, bots, spiders, scrapers or similar data gathering techniques and tools; (iii) use RPCI’s name, logo or trademarks. Material from the My Roswell Site may be downloaded, reproduced or transmitted only for User’s own personal noncommercial use, or if you are an RPCI health care provider, for use with patients; provided that (i) all copyright and other proprietary notices are kept intact, and (ii) no modifications are made to the materials. Except as expressly provided herein, RPCI does not grant any express or implied right to Users under any patents, copyrights, trademarks or trade secret information. RPCI may terminate this license and User rights to use the Site for any reason at any time. My Roswell is hosted in the United States, and Users may not use the Site in violation of United States export laws and regulations
The My Roswell site may not be accessed by persons under the age of 13.
My Roswell is housed on a dedicated web server, separate from the server that houses RPCI’s electronic medical record. To provide additional security, your session on My Roswell will “time out” or close after a period of inactivity of 11 minutes.
Safeguarding patient information requires the efforts of Users as well, and RPCI cannot be held responsible for unauthorized disclosures resulting from events outside of RPCI’s control, such as: a User not logging out of an active session; lost or stolen User ID/password; loss of information printed by the User; personal computer settings or installed software that may compromise information security.
Patients may disenroll in access to My Roswell by completing the online request form. Patients may also withdraw authorization for any or all Authorized Users by completing the online request form.
Users should visit this page from time to time to review the then-current My Roswell Terms because they are binding on Users. Certain provisions of the Terms may be superseded by expressly designated legal notices given to Users or terms located on particular pages of My Roswell. RPCI also reserves the right at any time and in its sole discretion to modify or discontinue, temporarily or permanently, the My Roswell Site (or any part thereof) with or without notice. User agrees that RPCI will not be liable to User or to any third party for any modification, suspension or discontinuance of My Roswell.
These Terms will be governed by and interpreted according to the laws of the State of New York, without regard to conflicts of law principles. Use of the Site is unauthorized in any jurisdiction that does not give effect to all provisions of these terms and conditions, including without limitation this section.
User recognizes and acknowledges that a breach by User of any of User’s obligations under the Terms will cause RPCI irreparable damage, which cannot be readily remedied by monetary damages in an action at law. Accordingly, in the event of any default or breach by User, including any action by you that could cause some loss or dilution of our goodwill, reputation, or rights in My Roswell, RPCI will be entitled to an immediate injunction in addition to any other remedies available, to stop or prevent such irreparable harm, loss, or dilution.
These Terms embody the entire agreement between User and RPCI with respect to use of My Roswell and supersedes all prior oral or written agreements and understandings relating to the subject matter of the Terms. No statement, representation, warranty, covenant or agreement of any kind not expressly set forth in the Terms will affect, or be used to interpret, change or restrict, the express terms and provisions of the Terms.
Fundraising: We may disclose certain information to our foundation (the Roswell Park Alliance Foundation) so that the Foundation may contact you for fundraising efforts. The information released would only be contact information, such as your name, address, phone number and the dates you received treatment or services at Roswell. If you do not want Roswell to contact you for fundraising efforts, you should notify the Roswell Park Alliance Foundation in writing to the Senior Gift Processor, c/o Roswell Park Cancer Institute, Elm and Carlton Streets, Buffalo, NY 14263, or call the Foundation at (716) 845-4444.
Healthcare Related Messages and Educational Events: We may use and disclose your health information in order to contact you with healthcare related messages (e.g., annual screening reminders) and provide you with information about our educational and supportive events and resources that may be of interest to you. If you do not want Roswell to contact you for these efforts you may unsubscribe through the My Roswell Portal or contact the My Roswell Portal Help Desk at 1-800-ROSWELL.
