MyRoswell provides important information from your medical record for you. You may also use MyRoswell to ask questions related to your health, your appointments, and your account. You may give access to Authorized Users, and this access can be discontinued, if you change your mind. It is very important that your login be unique to you and that any Authorized User(s) be set up with their own separate login. If you will not be accessing MyRoswell yourself, we can assist you in setting up Authorized User(s).
If you allow access to your information by your Authorized User(s), you do so at your own risk. Authorized User(s) will have access to sensitive information or test results. Any user of MyRoswell agrees to the terms and conditions
Do not use MyRoswell for urgent health needs. For emergency care, either go directly to the emergency department of a local hospital or call 9-1-1. For non-emergency care, call your health care provider directly. If you are calling after clinic hours, call 1-716-845-2300.

Terms and Conditions
Information provided through MyRoswell is provided under the terms and conditions provided below. If you are a patient of Roswell Park Comprehensive Cancer Center (RP), you may give access to your Authorized Users to view your health information. In the Terms and Conditions below, the term Users includes patients, their Authorized Users, members of your healthcare team and others who access MyRoswell for your treatment and healthcare operations. If you are a patient of Roswell Comprehensive Cancer Center you may also receive communication about research opportunities or about research that you are participating in, but you may choose to opt-out of receiving that communication. BY USING MYROSWELL, EACH USER AGREES TO COMPLY WITH THE FOLLOWING TERMS AND CONDITIONS. THESE TERMS AND CONDITIONS MAY BE CHANGED FROM TIME TO TIME, AND USE OF THE SITE IS CONDITIONED ON USER'S ACCEPTANCE OF THOSE TERMS AND CONDITIONS AS CHANGED. USER AGREES THAT RP IS NOT RESPONSIBLE OR LIABLE FOR ANY DIRECT OR INDIRECT CLAIM, LOSS OR DAMAGE RESULTING FROM HIS OR HER USE OF THE MYROSWELL SITE.
Information Provided
The information provided on MyRoswell includes general health information and information included in the patient's RP medical records, as well as to communicate non-urgent questions to RP personnel. The information on MyRoswell is made available for patients and their Authorized Users to share with health care providers now and in the future to improve understanding of health conditions. The information is not intended as a tool for self-diagnosis, is not a recommendation of a specific treatment plan or healthcare provider, and is not a substitute for proper medical advice, diagnosis, or treatment. Your doctor or other licensed health care provider is in the best position to assess individual health care needs and use of this site cannot replace medical consultation with a qualified health or medical professional. Information on MyRoswell is provided as is without warranty of any kind.
The information displayed in MyRoswell is not the complete medical record. Patients who need complete copies of the medical records can contact RP's Health Information Management Department and complete a release of information request form.
Site Use and Security
Users are granted a limited, revocable license to access and make use of the MyRoswell site only for personal use in accordance with these Terms. Users may not: (i) create derivative works using the MyRoswell site or its contents; (ii) use data mining tools, bots, spiders, scrapers, or similar data gathering techniques and tools; (iii) use RP's name, logo, or trademarks. Material from the MyRoswell Site may be downloaded, reproduced, or transmitted only for User's own personal noncommercial use, or if you are an RP health care provider, for use with patients; provided that (i) all copyright and other proprietary notices are kept intact, and (ii) no modifications are made to the materials. Except as expressly provided herein, RP does not grant any express or implied right to Users under any patents, copyrights, trademarks, or trade secret information. RP may terminate this license and User rights to use the Site for any reason at any time. MyRoswell is hosted in the United States, and Users may not use the Site in violation of United States export laws and regulations
The MyRoswell site may not be accessed by persons under the age of 12.
MyRoswell is housed on a dedicated web server, separate from the server that houses RP's electronic medical record. To provide additional security, your session on MyRoswell will time out or close after a period of inactivity of 11 minutes.
Safeguarding patient information requires the efforts of Users as well, and RP cannot be held responsible for unauthorized disclosures resulting from events outside of RP's control, such as: a User not logging out of an active session; lost or stolen User ID/password; loss of information printed by the User; personal computer settings or installed software that may compromise information security.
Other Terms
MyRoswell Only the named patient should access MyRoswell using the patient login credentials. If it is determined that someone other than the patient is accessing MyRoswell we will contact the patient to determine if that user should be converted to an Authorized User or if their access should be terminated. Patients may withdraw authorization for any or all Authorized Users in the portal or by contacting the Helpdesk.
Users should visit this page from time to time to review the then-current MyRoswell Terms because they are binding on Users. Certain provisions of the Terms may be superseded by expressly designated legal notices given to Users or terms located on particular pages of MyRoswell. RP also reserves the right at any time and in its sole discretion to modify or discontinue, temporarily or permanently, the MyRoswell Site (or any part thereof) with or without notice. User agrees that RP will not be liable to User or to any third party for any modification, suspension, or discontinuance of MyRoswell.
These Terms will be governed by and interpreted according to the laws of the State of New York, without regard to conflicts of law principles. Use of the Site is unauthorized in any jurisdiction that does not give effect to all provisions of these terms and conditions, including without limitation this section.
User recognizes and acknowledges that a breach by User of any of User's obligations under the Terms will cause RP irreparable damage, which cannot be readily remedied by monetary damages in an action at law. Accordingly, in the event of any default or breach by User, including any action by you that could cause some loss or dilution of our goodwill, reputation, or rights in MyRoswell, RP will be entitled to an immediate injunction in addition to any other remedies available, to stop or prevent such irreparable harm, loss, or dilution.
These Terms embody the entire agreement between User and RP with respect to use of MyRoswell and supersedes all prior oral or written agreements and understandings relating to the subject matter of the Terms. No statement, representation, warranty, covenant, or agreement of any kind not expressly set forth in the Terms will affect, or be used to interpret, change, or restrict, the express terms and provisions of the Terms.
Fundraising: We may disclose certain information to our foundation (the Roswell Park Alliance Foundation) so that the Foundation may contact you for fundraising efforts. The information released would only be contact information, such as your name, address, phone number and the dates you received treatment or services at Roswell. If you do not want Roswell to contact you for fundraising efforts, you should notify the Roswell Park Alliance Foundation in writing to the Senior Gift Processor, c/o Roswell Park Cancer Institute, Elm and Carlton Streets, Buffalo, NY 14263, or call the Foundation at (716) 845-4444.
Healthcare Related Messages and Educational Events: We may use and disclose your health information in order to contact you with healthcare related messages (e.g., annual screening reminders) and provide you with information about our educational and supportive events and resources that may be of interest to you. If you do not want Roswell to contact you for these efforts you may unsubscribe through MyRoswell or contact the MyRoswell Help Desk at 1-800-ROSWELL.
